Mechanism
Thyroid hormones regulate basal metabolic rate and oxygen consumption in nearly all cells, including those in the skin. In hypothyroidism, decreased T3 and T4 levels lead to reduced skin cell turnover, decreased sebum and sweat production, and the accumulation of mucopolysaccharides in the dermis, resulting in dry, coarse skin and non-pitting edema (myxedema). Conversely, hyperthyroidism increases metabolic rate, leading to warm, moist, and often smooth skin due to increased blood flow and sweating. The hormones exert their effect by binding to nuclear receptors in keratinocytes and fibroblasts, modulating gene expression for cellular proliferation and matrix synthesis.
