Testosterone-Induced Azoospermia is a reversible clinical state characterized by the complete absence of sperm in the ejaculate, which is directly caused by the exogenous administration of testosterone or other anabolic-androgenic steroids. This iatrogenic condition is a direct consequence of the pharmacological suppression of the hypothalamic-pituitary-gonadal (HPG) axis, leading to a profound failure of spermatogenesis. It is a critical and expected side effect that must be thoroughly discussed with all men of reproductive age considering Testosterone Replacement Therapy (TRT).
