Testosterone and SHBG

Meaning

Testosterone is the primary male sex hormone, an androgen crucial for muscle mass, bone density, libido, and erythropoiesis, produced mainly in the testes and ovaries. Sex Hormone-Binding Globulin (SHBG) is a glycoprotein synthesized primarily in the liver that binds to and transports sex hormones, including testosterone, estrogen, and dihydrotestosterone, in the bloodstream. The clinical interplay between these two is vital because only the unbound, or “free,” fraction of testosterone is biologically active and capable of engaging androgen receptors. Measuring both total testosterone and SHBG allows for the calculation of free testosterone, which is the most physiologically relevant measure.
