The clinical and patient-reported apprehension regarding the reduction in the size and functional capacity of the testes, which is a common, predictable side effect of long-term exogenous androgen administration. This atrophy results from the profound suppression of the hypothalamic-pituitary-gonadal (HPG) axis and the resulting lack of Luteinizing Hormone (LH) stimulation to the Leydig cells.
