A clinical treatment protocol for hypothyroidism involves administering both levothyroxine (T4, the prohormone) and liothyronine (T3, the active hormone) simultaneously to the patient. This combination approach aims to mimic the natural physiological secretion of the thyroid gland more closely than T4 monotherapy, which relies entirely on variable peripheral conversion. Combination therapy is often utilized when patients exhibit persistent hypothyroid symptoms despite having normalized TSH levels on T4-only treatment, suggesting a potential defect in T4-to-T3 conversion.
