The difference in biological activity, metabolic pathway, and clinical outcome observed when comparing a synthetic hormone compound to the corresponding endogenous hormone, arising from a deliberate alteration in its molecular structure. This deviation means the synthetic analog, or progestin, often exhibits altered receptor binding affinity, variable tissue selectivity, and distinct downstream metabolic products compared to the natural hormone. Clinically, this structural difference can lead to a non-physiological response and a different risk/benefit profile.
