Secondary Polycythemia

Meaning

Secondary Polycythemia is a clinical condition characterized by an elevated red blood cell mass resulting from an appropriate or inappropriate increase in the hormone erythropoietin (EPO), rather than a primary bone marrow disorder. This condition is termed ‘secondary’ because it is a compensatory response to an underlying physiological stimulus, most commonly chronic tissue hypoxia, such as living at high altitude or chronic obstructive pulmonary disease. However, it can also be an inappropriate response due to EPO-secreting tumors or, critically in the hormonal health space, a common side effect of exogenous testosterone replacement therapy. Clinically, managing this condition requires addressing the underlying cause and often involves therapeutic phlebotomy to mitigate the risk of thrombotic events due to increased blood viscosity.
