This concept is rooted in endocrinology, specifically the study of the anterior pituitary gland and its regulation by the hypothalamus. The discovery of dopamine’s role as the primary inhibitory factor for prolactin release provided the pharmacological basis for management. Clinical recognition of hyperprolactinemia as a cause of secondary hypogonadism established the necessity of active prolactin control.
Mechanism
Prolactin secretion is tonically inhibited by dopamine released from the hypothalamus into the portal system connecting to the pituitary gland. Management strategies, therefore, often involve the use of dopamine agonists to suppress prolactin release directly from the lactotroph cells. Chronic psychological or physical stress can also transiently elevate prolactin, necessitating the management of the HPA axis through rest and stress reduction protocols. By maintaining a balanced dopaminergic signal, the pituitary’s overproduction of prolactin is controlled, allowing the HPG axis to function optimally and sex hormone levels to normalize.
