Often referred to as the “mother hormone,” it sits at the apex of the steroidogenesis cascade, from which progesterone, DHEA, cortisol, testosterone, and estrogens are ultimately derived. Its circulating levels are often used as an indicator of overall adrenal and gonadal steroid production capacity.
Origin
The term is derived from the Greek pro-, meaning “before,” and pregnane, referring to the chemical structure of the steroid backbone found in progesterone, which is the immediate downstream metabolite. Pregnenolone was identified and characterized early in the study of steroid biochemistry, recognizing its pivotal role as the first step in the entire steroid hormone synthesis pathway.
Mechanism
The synthesis of pregnenolone is initiated within the mitochondria of steroidogenic cells when the enzyme cholesterol side-chain cleavage enzyme, or P450scc, converts cholesterol into pregnenolone. This reaction is the rate-limiting step in steroid hormone production, and it is regulated by trophic hormones such as ACTH in the adrenal cortex. Once formed, pregnenolone can be shunted into various metabolic pathways, either converting to progesterone via 3β-hydroxysteroid dehydrogenase or to DHEA via 17α-hydroxylase, thereby dictating the ultimate profile of downstream steroid hormones.
