Peak and Trough refers to the maximum and minimum concentrations, respectively, of a therapeutic agent in the bloodstream following its administration, a concept central to therapeutic drug monitoring (TDM). The peak concentration (Cmax) reflects the highest drug level achieved, often correlating with maximum therapeutic effect and potential for acute toxicity. Conversely, the trough concentration (Cmin) is the lowest level immediately before the next dose, serving as a critical indicator of whether the drug concentration remains above the minimum effective concentration (MEC) to sustain efficacy. Monitoring these values is essential for optimizing dosing regimens, particularly for hormones or drugs with a narrow therapeutic index.
