The Ominous Octet is a clinical term, originally coined by Dr. Ralph DeFronzo, that identifies the eight key pathophysiological defects in eight distinct organs or tissues that synergistically contribute to the development and progression of hyperglycemia in Type 2 Diabetes Mellitus. These defects involve a complex interplay of hormonal resistance and dysfunction, encompassing impaired insulin secretion from the beta-cells of the pancreas and increased hepatic glucose production. Recognizing this octet moves clinical focus beyond the single defect of insulin resistance to a comprehensive, multi-organ endocrine and metabolic failure, necessitating poly-pharmacological intervention.
