Neoantigens are novel protein fragments, or peptides, that arise from somatic mutations within cancer cells and are not present in normal, healthy tissues. These unique structures represent distinct molecular signatures specific to an individual’s tumor, making them ideal targets for a highly personalized immune response.

Context

Within the intricate landscape of human immunology and oncology, neoantigens function as critical identifiers presented on the surface of tumor cells by Major Histocompatibility Complex (MHC) molecules. This presentation allows the body’s immune surveillance system, particularly T lymphocytes, to recognize these altered cells as foreign, distinguishing them from the body’s own healthy cellular components.

Significance

The clinical importance of neoantigens lies in their exceptional specificity as targets for anti-tumor immunity. Identifying and characterizing neoantigens typically involves comprehensive genomic analysis, such as whole-exome sequencing of both tumor and normal tissue samples. The immunogenicity of predicted neoantigens can be further assessed through in vitro T-cell activation assays, measuring cytokine production or T-cell proliferation.

Risk

While offering significant therapeutic promise, challenges and risks associated with neoantigens include the technical complexity of their identification and synthesis, as well as the potential for tumor immune escape mechanisms where tumor cells may downregulate neoantigen expression.
