Maternal hypothyroxinemia is an endocrine condition in pregnant women characterized by low circulating levels of free thyroxine (fT4), the primary thyroid hormone, while the thyroid-stimulating hormone (TSH) levels remain within the normal reference range for gestation. This state signifies a deficiency in the essential thyroid hormone supply available to the developing fetus, particularly during the first half of pregnancy when the fetal thyroid gland is not fully functional. It is a distinct entity from overt hypothyroidism and is clinically significant due to its association with potential adverse neurodevelopmental outcomes in the offspring.
