Late Initiation MHT describes the clinical scenario where Menopausal Hormone Therapy (MHT) is commenced significantly later in a woman’s life, typically ten or more years after the onset of menopause or after the age of 60. This timing falls outside the conventionally defined “window of opportunity” and is associated with a different risk-benefit profile, particularly concerning cardiovascular health. While still effective for managing severe vasomotor symptoms, the initiation of systemic hormone therapy in this cohort requires heightened clinical caution due to the presence of pre-existing subclinical atherosclerosis.
