Intratesticular Testosterone

Meaning

Intratesticular testosterone refers to the concentration of the androgen testosterone specifically within the testicular tissue, which is significantly higher than the level found in the general systemic circulation. This extremely high localized concentration, often hundreds of times greater than plasma levels, is absolutely critical for the process of spermatogenesis, the production of viable sperm. When men undergo exogenous testosterone replacement therapy, the introduction of external testosterone often suppresses the pituitary hormones (LH and FSH), leading to a drastic reduction in this essential intratesticular concentration, which can cause infertility. Clinical management must consider this distinction when treating hypogonadism in men desiring to maintain fertility.
