Insulin Therapy in Pregnancy is the standard pharmacological intervention for managing hyperglycemia in women with pre-existing diabetes or gestational diabetes mellitus (GDM) when dietary and lifestyle modifications are insufficient to achieve target glucose levels. This treatment is critical for maintaining strict maternal glycemic control, as uncontrolled hyperglycemia can lead to serious adverse fetal outcomes, including macrosomia, neonatal hypoglycemia, and an increased risk of congenital malformations. Insulin is the preferred agent because it does not cross the placental barrier to a significant degree.
