The Hormone Therapy Timing Hypothesis proposes that the ultimate balance of therapeutic benefits versus risks associated with hormone replacement therapy (HRT) is critically dependent on the patient’s age and the duration since the onset of menopause. Specifically, this clinical principle suggests that HRT initiated closer to the final menstrual period, often termed the “critical window,” is associated with a more favorable cardiovascular risk profile and overall health outcomes. Conversely, initiating therapy many years post-menopause, after significant vascular aging, may carry greater potential risks.
