A metaphorical concept describing the natural tendency of an individual’s accumulated health data—including physiological metrics, genetic results, and clinical history—to become fragmented, disorganized, inconsistent, or inaccessible over time. High entropy in health information reduces its clinical utility, making it difficult to construct an accurate, holistic patient profile for personalized treatment planning. It represents a loss of valuable clinical signal.
