Origin
The term originates from the ultracentrifugation techniques used in the mid-20th century to separate plasma lipoproteins based on their density, where HDL was found to be the fraction with the highest density. The designation ‘cholesterol’ is functional, referring to the cholesterol esters carried within the lipoprotein. The clinical recognition of its protective role solidified its importance in preventative medicine.
Mechanism
The primary protective mechanism of HDL is reverse cholesterol transport, a process where nascent HDL particles acquire free cholesterol from cells, including foam cells in arterial walls, using the ABCA1 transporter. Lecithin-cholesterol acyltransferase (LCAT) then esterifies this cholesterol, trapping it within the HDL core. The mature HDL particle returns to the liver, where the cholesterol is selectively taken up for bile acid synthesis and excretion, effectively reducing the overall peripheral lipid burden and mitigating plaque formation.
