Gonadal Axis Restoration is the clinical process aimed at re-establishing or optimizing the normal feedback loop between the hypothalamus, pituitary gland, and the gonads (testes or ovaries). This axis controls the pulsatile release of GnRH, LH, and FSH, which in turn regulate endogenous sex hormone production, such as testosterone or estradiol. Restoration is necessary following suppression due to exogenous hormone use, chronic stress, or age-related decline in gonadotropin output. The ultimate clinical goal is achieving endogenous reproductive hormone sufficiency.
