The term glymphatic is a portmanteau combining glia (referring to astrocytes, which facilitate the process) and lymphatic (due to its functional similarity to the peripheral lymphatic system). The system’s existence and function were only recently discovered, revolutionizing the understanding of brain fluid dynamics. Clearance rate is a physiological metric adapted to assess the system’s detoxification efficiency, emphasizing the flow dynamics of cerebrospinal fluid.
Mechanism
During sleep, the brain’s interstitial space expands significantly, and cerebrospinal fluid (CSF) flows rapidly along the periarterial spaces, driven by arterial pulsation. Astrocytes, through their aquaporin-4 channels, facilitate the bulk flow of CSF into the brain tissue, where it exchanges with interstitial fluid. The fluid, now containing waste products, is then channeled out along perivenous spaces, effectively clearing the metabolic debris accumulated during wakefulness. This process is largely dependent on the depth and continuity of sleep.
