Glucocorticoid Load Management is the clinical strategy focused on reducing the total cumulative exposure of tissues to active glucocorticoids, such as cortisol, over a given period. This load encompasses both endogenous production by the adrenal glands and exogenous intake from medications. Chronic high glucocorticoid load, even within the “normal” range, is associated with insulin resistance, bone density loss, and central fat deposition. The goal of management is to minimize these catabolic and metabolic risks by optimizing the production, metabolism, and receptor binding of these potent steroid hormones.
