Origin
Ghrelin was discovered in 1999 as the endogenous ligand for the growth hormone secretagogue receptor (GHS-R), which led to its name derived from the Proto-Indo-European root ‘ghre-‘ (to grow). The understanding of its role quickly expanded beyond growth hormone release to include its potent orexigenic (appetite-stimulating) function. The concept of ‘modulation’ arose from the clinical desire to pharmacologically influence this hunger signal.
Mechanism
Ghrelin, often termed the ‘hunger hormone,’ acts by binding to the GHS-R, which is highly expressed in the hypothalamus, the brain’s primary appetite control center. Binding stimulates the release of neuropeptide Y and Agouti-related peptide, thereby promoting food intake. Modulation mechanisms involve influencing the vagus nerve signaling or directly targeting the GHS-R with agonists or antagonists to either suppress appetite in weight management or stimulate it in conditions of insufficient nutrient intake.
