Functional Secondary Hypogonadism is a reversible clinical state characterized by low circulating sex hormone levels, such as testosterone, resulting from a suppression of the hypothalamic-pituitary-gonadal (HPG) axis due to systemic non-gonadal factors. Unlike primary hypogonadism, the testes or ovaries themselves are structurally intact; the issue lies with insufficient signaling from the pituitary and hypothalamus. Common triggers include chronic illness, severe stress, significant obesity, or excessive exercise, indicating a systemic adaptation rather than a primary endocrine failure.
