Functional Iron Deficiency

Meaning

Functional Iron Deficiency (FID) is a clinical state characterized by an insufficient rate of iron mobilization and utilization to meet the physiological demands of the body, particularly for effective erythropoiesis, despite the presence of adequate total body iron stores. In this scenario, storage iron, as reflected by serum ferritin levels, may be normal or even elevated, yet the transport iron is functionally unavailable, leading to impaired hemoglobin synthesis and cellular energy production. This condition is commonly observed in the context of chronic inflammation, which disrupts the iron regulatory pathway. Clinically, it is diagnosed by low transferrin saturation (TSAT) in the presence of normal or high ferritin.
