Free-to-Total PSA

Meaning

Free-to-Total PSA is a clinically utilized ratio calculated by dividing the concentration of unbound, or free, prostate-specific antigen (PSA) in the serum by the concentration of total PSA (free plus complexed forms). This ratio is employed as a diagnostic adjunct to the total PSA test, primarily to help differentiate between benign prostatic hyperplasia (BPH) and prostate carcinoma in men with mildly elevated total PSA levels. A lower percentage of free PSA is generally associated with an increased probability of prostate cancer, while a higher percentage is more typical of BPH. This measurement aids clinicians in deciding whether to recommend a prostate biopsy.
