Free Testosterone Monitoring is the clinical practice of measuring the biologically active, unbound fraction of testosterone in the circulation, which is not attached to sex hormone-binding globulin (SHBG) or albumin. This measurement provides a more accurate representation of the hormone available to interact with target tissue receptors and exert its physiological effects compared to total testosterone alone. It is a critical diagnostic tool for assessing androgen status, particularly in cases of suspected hypogonadism or androgen excess, where SHBG levels may be abnormally high or low. The free fraction is the true indicator of androgen action.
