Free Hormone Hypothesis

Meaning

The foundational endocrinological concept asserting that only the fraction of a hormone circulating unbound to carrier proteins, known as the “free hormone,” is biologically active and capable of diffusing into target tissues to interact with cellular receptors. Hormones tightly bound to plasma proteins like Sex Hormone-Binding Globulin (SHBG) or Thyroid-Binding Globulin (TBG) are generally considered inactive and serve as a circulating reservoir. This hypothesis is critical for interpreting total hormone measurements and understanding clinical hormonal status.
