The clinical measurement of the unbound, biologically active portion of a hormone circulating in the bloodstream, distinct from the fraction bound to transport proteins like Sex Hormone Binding Globulin (SHBG) or albumin. This assessment is considered a more accurate representation of the hormone’s true biological availability and its potential to exert effects on target tissues. Total hormone levels can be misleading if the concentration or binding capacity of these transport proteins is significantly altered.
