Meaning

The phenomenon of drug metabolism whereby the concentration of an orally administered compound is significantly reduced before it reaches the systemic circulation, primarily due to enzymatic breakdown in the liver. Following absorption from the gastrointestinal tract, the drug travels via the portal vein directly to the liver, which acts as the body’s primary metabolic filter. This effect dictates the bioavailability and required dosing of many orally delivered hormonal agents.
