Dosing Errors are any preventable event that occurs during the prescribing, dispensing, or administration of a medication that leads to or has the potential to lead to inappropriate medication use or patient harm. These errors include administering the wrong dose, an incorrect frequency, or an inappropriate route of administration for a prescribed hormonal therapy. Accurate dosing is paramount in endocrinology due to the narrow therapeutic indices of many hormones. Such errors can severely compromise the intended therapeutic effect.
