Delayed testosterone recovery is a clinical phenomenon characterized by the prolonged suppression of endogenous testosterone production following the cessation of exogenous androgen administration, such as in post-cycle therapy or discontinuation of testosterone replacement therapy (TRT). This state reflects a persistent dysregulation of the hypothalamic-pituitary-gonadal (HPG) axis, where the pituitary gland fails to rapidly resume adequate luteinizing hormone (LH) and follicle-stimulating hormone (FSH) signaling to the testes. Clinically, it presents as a period of secondary hypogonadism, often associated with symptoms of low energy, decreased libido, and mood disturbances. The duration of this delay is highly variable and depends on the previous dose, duration, and type of exogenous androgen used.
