Cortisol diurnal patterning describes the natural, cyclical variation in the secretion of the glucocorticoid cortisol over a 24-hour period. In a healthy individual, this pattern is characterized by a sharp rise shortly after waking, known as the Cortisol Awakening Response (CAR), followed by a gradual decline throughout the day, reaching its nadir late at night. Assessing this specific rhythm is a crucial diagnostic tool in endocrinology, offering insight into the integrity and adaptability of the hypothalamic-pituitary-adrenal (HPA) axis. Deviations from this optimal pattern often signal chronic stress or HPA axis dysregulation.
