The Cortisol Awakening Profile is a specialized diagnostic assessment that measures the dynamic change in salivary cortisol concentration over the first hour following waking. It specifically captures the Cortisol Awakening Response (CAR), which is a rapid, transient increase in cortisol secretion independent of the HPA axis’s circadian rhythm. Clinically, this profile provides critical insight into the individual’s stress axis function, allostatic load, and hypothalamic-pituitary-adrenal (HPA) axis integrity.
Origin
The term combines ‘cortisol,’ the primary human glucocorticoid, with ‘awakening profile,’ referring to the specific pattern of its release upon arousal. The CAR phenomenon was first described in the mid-20th century, and the profile has become a standard, non-invasive tool in psychoneuroendocrinology. Its origin is firmly rooted in the study of circadian and ultradian rhythms and their intersection with stress physiology.
Mechanism
The CAR is believed to be initiated by a signal from the Suprachiasmatic Nucleus (SCN), the brain’s master clock, which stimulates the paraventricular nucleus (PVN) of the hypothalamus immediately upon awakening. This signal triggers the release of Corticotropin-Releasing Hormone, which rapidly cascades to Adrenocorticotropic Hormone release from the pituitary, stimulating the adrenal glands to produce cortisol. A blunted or excessively high profile can indicate chronic stress, adrenal fatigue, or HPA axis dysregulation, signaling compromised hormonal resilience.
