Chronic Health Conditions are non-communicable diseases or disorders that persist for a long duration, typically defined as one year or more, and generally require ongoing medical attention or limit an individual’s activities of daily living. These conditions are characterized by their slow progression and are often irreversible, fundamentally altering a patient’s quality of life and placing a substantial burden on healthcare systems. Common examples include Type 2 Diabetes, cardiovascular disease, and chronic obstructive pulmonary disease.
