The term was coined in 2013, derived from “glia” (referring to the glial cells, particularly astrocytes, that drive the flow) and “lymphatic” (due to its analogous function to the peripheral lymphatic system). Mechanism
The glymphatic system functions by using the perivascular spaces as fluid conduits, driving cerebrospinal fluid (CSF) into the brain tissue and facilitating the exchange with interstitial fluid (ISF). Astrocytic endfeet, through the aquaporin-4 (AQP4) water channel, are the key regulatory elements that facilitate this bulk flow. During deep sleep, the interstitial space expands, significantly increasing the flow rate and enabling the efficient washout of metabolites, including amyloid-beta and tau proteins, which are implicated in neurodegeneration and chronic inflammation.
