Mechanism
The hypothalamus continuously monitors circulating levels of adipokines like leptin and ghrelin, alongside insulin, to gauge overall energy reserves. When body fat drops below the setpoint, a decrease in leptin triggers a neuroendocrine response that simultaneously increases hunger signals via NPY/AgRP and reduces total energy expenditure. Conversely, a high setpoint can be maintained by chronic leptin resistance, which dampens the satiety signal despite excessive energy stores.
