Bioavailable Estradiol Management is the clinical practice of optimizing the concentration of estradiol (E2) that is freely available to bind to target tissue receptors and elicit a biological response. This fraction includes unbound estradiol and estradiol loosely bound to albumin, excluding the fraction tightly bound to Sex Hormone-Binding Globulin (SHBG). Effective management aims to maintain therapeutic levels without inducing adverse effects associated with excessive or deficient estrogenic activity. It requires precise dosing and monitoring of both total estradiol and SHBG levels to assess true tissue exposure.
