Bioactive sex steroids represent the fraction of sex hormones, such as testosterone and estradiol, that are not bound to carrier proteins like Sex Hormone-Binding Globulin (SHBG). This unbound, or “free,” portion is biologically active, allowing it to readily diffuse into target cells to exert its physiological effects. Clinically, assessing this concentration provides a more accurate representation of the hormone available to interact with tissue receptors and drive biological responses than total hormone levels alone.
