A chemical modification process, 17-alpha-alkylation involves adding an alkyl group, typically a methyl group, to the 17-alpha position of a steroid molecule. This specific structural alteration is applied to orally administered synthetic androgens and progestins. The primary clinical purpose of this modification is to enhance the molecule’s resistance to rapid metabolic breakdown. This structural change prevents the steroid from being immediately inactivated by the liver upon its first pass through the hepatic circulation.
